Honeywell Safety Manager & I/O Module failures

We have SIL-3 certified Honeywell Safety Manager installed at site. A total of 6 each SMs are installed in redundant configuration i.e on controller as well as I/O levels. Since commissioning we have been facing frequent I/O module failures.

Experon Server & Network traffic

Few days back we have had an incident due to which all console stations went blank and restored after 12 mins. Interestingly no changeover of Experion servers occurred and Consoles did not picked up data from Controllers directly as well.

Network traffic and Server data lost

We have C-300 based Experion DCS installed at our plant with FTE network. System comprised of redundant Experion Servers and multiple Experion consoles & flex stations.
