I have a S5 135U from Siemens which gives QVZ (i/o) error on the CPU 928B intermittently, and the outputs starts blinking w/o actually the logic operating. This has caused serious damage to hydraulic motor contactors. When you see ladder the logic output is steady but it blinks on cards, and CPU, QVZ also blinks. CPU used is CPU928B.
When the program is washed and reloaded it works for few days without problem.

i think if you change (Q) in program or change output card your problems will be solved.
I had once incident when I must change on new a CPU 928B. The CPU 928B (with eprom) has a problem with running - go to run mode OK. Previous a Service Man made check a base plane (connection between card and a base plane, solder point ). Sometime the CPU working Ok, but few mounts leter a problem was again. Only helped change the CPU. Maybe You change your a CPU on another a proficient CPU?
